The clinical social worker provides psychosocial assessments and develops care transition plans to assist patients and families in coping with hospitalization and illness. They collaborate with healthcare teams and advocate for patients while ensuring timely and accurate documentation.
A Master's degree in Social Work is required, along with a minimum of two years of experience in a healthcare setting. Effective communication and interpersonal skills, along with advanced knowledge of social work theory and practice, are essential.
