The Registered Nurse acts as the hub of the care team, triaging communications and collaborating with providers to deliver evidence-based geriatric care. They also conduct virtual patient assessments, develop personalized care plans, and oversee post-appointment processes like diagnostic and medication orders.
Candidates must hold a current unrestricted RN license with at least 2 years of clinical experience, preferably in geriatrics. Strong proficiency in remote work, technology, and digital communication platforms is required.
