Our Hospice Visiting Nurse (RN) utilize their extensive education and training to provide personalized care to our patients in a rewarding environment that promotes autonomy.

Job Description

Support our patients, families, and loved ones, as they go through their end of life journey Perform a comprehensive assessment to establish goals for symptom management and end of life care with effective case management strategies Be a member of an active interdisciplinary team that addresses patients’ needs holistically Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families. Utilize state-of-the-art technology to document skilled services provided.

Registered Nurse with Valid CT RN license One year of RN experience; Homecare or Hospice experience preferred.

Valid driver’s license and reliable transportation required.

Computer proficiency required.

Requirements

Candidates must be a Registered Nurse with a valid CT RN license and have at least one year of RN experience, preferably in homecare or hospice. A valid driver's license and computer proficiency are also required.
