Salary
$55 - $58 / HOUR
The Registered Nurse will deliver evidence-based care to seriously ill patients by following individualized care plans and conducting patient assessments. They will also facilitate care coordination, manage patient visits, and educate caregivers on palliative care and symptom management.
Candidates must hold an active, unrestricted California Registered Nurse license and have at least 2 years of clinical nursing experience in home or community health. A valid driver's license, reliable vehicle, and proficiency in EMR and Google Suite are also required.

This field-based position requires traveling throughout Solano County and surrounding areas. Travel requirement is within a 100-mile radius of Solano County. This is a critical role and we're looking to fill it as soon as possible.
Bachelor of Science in Nursing (BSN). CHPN certification or equivalent.
