Provide one-on-one physical therapy care to patients in their homes or senior living facilities to restore functionality and movement. Develop individualized care plans, document patient progress, and coordinate with an interdisciplinary team.
Requires an active physical therapy license in good standing and a valid driver's license. CPR certification is also required, which the company can provide if necessary.
Provide one-on-one care to restore functionality by improving movement and range of motion for elderly, critically ill, disabled, those recovering from a surgery, injury, or accident, or a combination. Review physician referrals, assess conditions, and develop an individualized plan of care designed to meet patient-centered goals. Coordinate care with an interdisciplinary team by documenting patient condition, progress, and education provided to patient and family/caregiver. Supervise delegation of work performed by PTA.
What you will need: · Current active license in good standing with the state of employment
