Role Overview A forward-thinking telehealth organization is seeking a Pharmacist to support patients across Hawaii through remote clinical services. Key Responsibilities Provide comprehensive Medication Therapy Management (MTM) services Conduct medication reconciliation and assess therapy effectiveness Deliver patient-focused education on medications and chronic conditions Support chronic disease management (e.g., diabetes, hypertension, COPD, CHF) Utilize motivational interviewing techniques to drive adherence and engagement Collaborate with providers to optimize medication regimens and care plans Identify and resolve medication-related issues and risks Document patient interactions accurately and in compliance with program standards Support care transitions and continuity of care initiatives
Required Qualifications Doctor of Pharmacy (PharmD) Active pharmacist license in Hawaii (HI) Willingness to obtain additional state licensure if needed
Board certification (e.g., BCPS, BCGP) strongly preferred Experience in: Medication Therapy Management (MTM) Ambulatory care or outpatient settings Chronic disease management Patient education and engagement Familiarity with care programs such as CCM, TCM, and RPM is a plus
