The Registered Nurse will conduct in-depth clinical assessments and develop individualized care plans for older adults. They will also partner with an interdisciplinary team to coordinate wraparound care and provide training on medication safety and disease management.
Candidates must hold an active Pennsylvania Registered Nurse license and possess at least one year of experience working with older adults or medically complex individuals. A valid driver's license, reliable vehicle, and CPR certification are also required.

You’ll serve older adults throughout Allegheny County, providing care at our Day Health Centers and in Participants’ homes, community spaces, or local healthcare facilities—wherever support is needed most.
What You’ll Need Active PA Registered Nurse license Minimum one year of experience working with older adults or medically complex individuals CPR certification Valid driver’s license, reliable vehicle, and auto insurance This is more than a nursing job.
