Job Summary

Fox Subacute at Philadelphia is seeking a highly motivated Physical Therapist (PT) to join our interdisciplinary care team. Our 53-bed subacute facility specializes in the care of clinically complex respiratory and ventilator-dependent residents, delivering hospital-level acuity in a supportive, home-like environment.

  • Key Responsibilities
  • Develop and implement individualized physical therapy treatment plans based on patient assessments and care goals
  • Deliver high-quality, evidence-based physical therapy interventions
  • Monitor patient progress and adjust treatment plans as needed
  • Collaborate closely with physicians, nurses, respiratory therapists, and rehabilitation staff to optimize patient outcomes
  • Maintain accurate and timely documentation in accordance with regulatory and facility requirements
  • Uphold the highest professional and ethical standards of physical therapy practice

Qualifications

  • & Requirements
  • Current Physical Therapist license in the state of Pennsylvania (required)
  • Experience in subacute, long-term care, or respiratory care preferred (new graduates encouraged to apply)
  • Strong clinical assessment and treatment planning skills
  • Ability to work effectively with a specialized, medically complex patient population
  • Excellent communication and teamwork skills
  • Ability to work PRN in person hours- One shift per month minimum, one shift per pay period preferred
