We are currently seeking a Full Time Case Manager for our Partial Care department located in Jersey City, New Jersey. Partial Care is a day treatment program for severely and persistently mentally ill patients who require structure, intensive treatment, and close medication monitoring to prevent hospitalization.
The Case Manager provides case management and support to Partial Care clients, facilitates groups, documents according to regulatory standards, carries a caseload, interfaces with Partial Care psychiatrist and maintains collateral contacts with family members and providers in the community on behalf of clients.
