The case manager will evaluate behavioral health surveys, identify socio-economic and medical risk factors, and provide necessary referrals for prenatal patients. They will work alongside nursing staff to triage visits and assist families with essential resources like food, formula, and pediatric care coordination.
Candidates must hold a Bachelor's degree in Social Work or a related field and possess at least two years of case management experience. Required certifications include BLS and Mandatory Reporter status, along with the ability to address cultural and socio-economic barriers to care.
Prenatal behavioral health case manager: 1– Full-time M-Fri 8am-5pm
