The Case Manager coordinates individualized services to maximize consumer recovery and independence for individuals with mental illness. Responsibilities include conducting psychosocial assessments, facilitating person-centered planning, and coordinating care with community resources and hospital providers.
A bachelor's degree in a human services field is required, with professional licensure (LBSW, LMSW) preferred. Candidates must have a valid Michigan driver's license and prior case management experience in a mental health or rehabilitation setting is preferred.
The Case Manager coordinates all services necessary to maximize consumer recovery and independence for individuals with mental illness including implementing strategies for obtaining services and supports that are goal-oriented and individualized.
