About the role

Provide professional nursing care to patients in their homes or alternate-site settings according to nursing practices and agency policies. Collaborate with the interdisciplinary team to develop, implement, and document individualized plans of care for patients and their families.

What you can expect

Location: Territories varied through 9 counties in central NC

Our Hospice team at Duke HomeCare and Hospice delivers compassionate, patient-centered care to individuals and families during critical moments. In this evening shift role, you will support patients in home-based settings, helping ensure comfort, dignity, and high-quality care at the end of life.

Schedule

• (5) 8 hour evening shifts (1 p.m.-9 p.m.), Monday to Friday

How you will make an impact

Provide professional hospice nursing care in home or alternate settings in accordance with North Carolina nursing regulations and agency standards.

Assess patient conditions, manage symptoms, and communicate significant changes to physicians and interdisciplinary care teams.

Educate patients and families on disease progression, medications, and end-of-life care to promote comfort and understanding.

Collaborate with interdisciplinary teams to coordinate care plans, support discharge planning, and ensure quality outcomes.

Maintain accurate documentation and ensure compliance with regulatory, safety, and quality standards.

What you will need

Associate Degree or Diploma in Nursing required; BSN encouraged after two years of service.

Current RN license in North Carolina or compact state.

CPR certification (American Heart Association).

Hospice experience required for this evening shift position.

Valid driver’s license and current automobile insurance coverage.

Two years of med-surgical, critical care, home health, or hospice experience preferred.

Strong assessment, communication, and documentation skills with the ability to work independently in home settings.

Requirements

Requires an Associate Degree or Diploma in Nursing and a current North Carolina RN license. Candidates must have a valid driver's license, current automobile insurance, and a current CPR certification.
