About the role

The role involves managing a caseload of clients by designing and implementing skill training groups and maintaining accurate case records. It also requires coordinating with inter-agency staff and assisting clients with community service referrals and transportation.

Description The Mental Health Support Worker (Psychiatric Rehabilitation Department) is responsible for a caseload of clients; for designing, developing and implementing skill training groups, modifying the curriculum as needs change, while meeting state regulations with regard to charting requirements.

Requirements: Essential duties and responsibilities

1.

Assume responsibility for assigned caseload, which includes presenting cases to the team.

2.

Identify client needs/problems, strengths, goals and developing a plan to meet the goals as part of the planning process.

3.

The compilation of and maintenance of timely and accurate case records on assigned clients which include daily progress notes on clients, service plan reviews, appropriate referral forms, chart audits, intake and other forms as needed.

4.

Assume the role of group leader in group sessions and other skill training as assigned.

5.

Establish and maintain communication with other NTC treatment staff or staff from other agencies regarding mutual clients when it is appropriate and necessary to do so to meet the needs of the client.

6.

Attend and participate in inter-agency meeting regarding mutual clients as needed.

7.

Assist in referring assigned clients to other needed community services.

8.

Participate in Staff Development activities to enhance skills and education.

9.

Utilize NTC’s Electronic Health Records (E.H.R.) during tenure of employment.

Initial training, and on-going training, will be provided to employee by respective supervisor and/or assigned team member.

10.

Assist assigned clients in accessing transportation through the Endless Mountains Transportation Authority.

11.

PRS Worker must complete 18 hours of training per calendar year, with 12 hours being specific to Psychiatric Rehabilitation.

12.

PRS worker must have six (6) hours of shadowing on the Psych Rehab Floor.

13.

Staff member may be requested to transport clients to and from program when transporter(s) are unavailable or scheduled off.

14.

Staff member, at the manager’s discretion, will be assigned to do weekend day trip(s) when scheduled for the consumers.

15.

Attends weekly staff meetings.

16.

Attend general staff meetings as scheduled.

17.

Participates in ongoing in-service training programs.

18.

Employee will be proficient in their job position within six (6) months.

If at any time an employee feels that they need more training/education, employee is to submit a request for such through their supervisor, manager, and/or director.

19.

Other functions and duties as assigned.

EDUCATION and/or EXPERIENCE 1.

High school diploma or equivalent with two (2) years of experience in the mental health field.

Comfort level with Crisis Intervention.

2.

Some knowledge of the major diagnostic categories of mental illness.

3.

Strong interpersonal and communication skills as well as ability to work effectively as a team member.
