The Case Manager is responsible for coordinating patient care, treatment planning, and crisis intervention services to ensure a seamless continuum of care. They also facilitate linkages to community resources and conduct educational sessions for patients and their families.
Candidates must hold a Bachelor’s degree in a Human Service field and possess a valid driver's license. Previous experience in case management or utilization review is preferred.
