To demonstrate support of the hospital mission and vision by providing nursing, case management/disease management services to clients and families to help in the coordination and management of individuals’ health needs through the full continuum, as needed. The Centers Registered Nurse demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care while ensuring and facilitating the achievement of optimal quality, clinical and financial outcomes. Performs assessment of clinical and life issues for the patients of City of Hope. Provides professional nursing services in support of the physician, in the direct and indirect care of patients.

• Communication: Timely and thorough documentation of assessments, interventions, and patient education in the electronic medical record is crucial. The nurse communicates effectively with the healthcare team and patients to ensure coordinated care.

• Support: The nurse provides patient education, supports informed decision-making regarding treatment options, and ensures access to ancillary services and resources.

Qualifications

Your qualifications should include

• Current Georgia RN license from an accredited college
• BSN required or enrolled within 6 months of hire and BSN completed within three years from hire date
• BLS (Obtain AHA certification within 30 days of hire)
