$24–$26 per hour Alhambra Post Acute in Martinez, California is seeking a compassionate and organized Social Services Designee to join our team. In this role, you will support residents and their families by helping address social, emotional, and psychosocial needs while promoting the highest possible level of well-being. If you are empathetic, proactive, and passionate about advocating for residents, we would love to meet you. Responsibilities Provide medically related social services to help residents maintain the highest practicable physical, mental, and psychosocial well-being. Evaluate residents’ social, emotional, and family needs and assist in developing individualized care plans. Provide emotional support and counseling to residents and families coping with illness, disability, grief, or life changes. Assist residents and families with healthcare decisions and care planning. Coordinate with the interdisciplinary team on discharge planning, including referrals for community services and equipment needs. Schedule outpatient appointments and ancillary services such as optometry, podiatry, dental, and psychiatric visits. Assist residents in obtaining community resources and support services. Help with personal shopping and tracking resident belongings, including follow-up on lost or missing items. Maintain accurate documentation related to resident social service status. Build strong relationships with facility departments and community agencies to support resident needs.
