The Nurse Practitioner (NP) in this role will deliver comprehensive, person-centered primary and palliative care services to seniors through home-based clinical settings. The NP will focus on quality of life, chronic disease management, and health promotion—aligned with the mission of Snowline Health and the needs of our community. As part of a multidisciplinary care team, the provider will coordinate care, develop robust treatment plans, and guide complex care transitions to support program participants and their families.
Salary Range: $65–$80 per hour
Clinical Care Deliver in-home care and develop comprehensive care plans for medically complex patients Conduct health assessments and physical examinations Diagnose and manage chronic conditions Prescribe medications and treatments as appropriate Educate patients and caregivers on health management Document accurately and promptly in the EMR Participate in provider on-call rotation Care Coordination Collaborate with multidisciplinary teams Serve as a key contact for patients and families Coordinate care transitions and specialist referrals Program Development Help develop and refine clinical processes and workflows Support quality improvement initiatives Provide mentorship and support to clinical team members Regulatory Compliance & Quality Assurance Ensure compliance with relevant laws, regulations, and organizational policies Maintain knowledge of geriatric, primary, and palliative care trends Stay current with CMS billing and coding for home-based care Community Leadership & Engagement Represent Snowline ElderCare in the community Serve as a subject matter expert for elder care best practices Mentor students and emerging clinical team members
Current and valid California NP license Minimum 3 years’ clinical experience in geriatrics, palliative care, or primary care Proficiency in EMRs, HIPAA compliance, and medical terminology Experience in home-based care delivery preferred Strong assessment and diagnostic skills Excellent interpersonal, communication, and time management skills Ability to work independently and as part of an interdisciplinary team Valid CA driver’s license and insured vehicle for home visits Active DEA registration and CURES compliance Knowledge of trauma-informed care a plus
