The Clinical Pharmacist will optimize patient medication therapy, conduct regimen reviews, and provide clinical support to healthcare staff. They will also manage disease-specific education for conditions like diabetes and CHF while participating in multidisciplinary patient care rounds.
Candidates must hold a Doctor of Pharmacy (Pharm.D.) degree and possess a current pharmacist license in the State of California. Strong organizational, communication, and independent judgment skills are required for this role.

• Participate and maintain a significant role in multidisciplinary patient care rounds
• Participate in different activities of outreach program to talk about the importance of medication management
• Provide support to patients and physicians when medication is recommended and prescribed but not covered by patient’s health plan pharmacy benefit and lack the financial resources to pay for their medications and co pays
The pay range for this position at commencement of employment is expected to be between $125,000 - $130,000 annually; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.
The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.
