The Clinical Pharmacist provides patient care by optimizing medication regimens, conducting reviews, and counseling patients to ensure safe and effective outcomes. They also collaborate with multidisciplinary teams to manage transitions of care and support disease management programs for conditions like diabetes and CHF.
Provides patient care that optimizes the use of medication and promotes health, wellness, and disease prevention.
Reviews all medications and ensure accuracy and effective functioning and manage all communication with physicians and patients and assist to resolve all patients within required timeframe.
Involved in the education of patients about new medications and in the education of all patients about their discharge medications
Discover and awareness of discrepancies, and to provide a safe and effective process for obtaining, documenting, and communicating medications across a patient’s continuum of care
• Participate and maintain a significant role in multidisciplinary patient care rounds
• Participate in different activities of outreach program to talk about the importance of medication management
• Provide support to patients and physicians when medication is recommended and prescribed but not covered by patient’s health plan pharmacy benefit and lack the financial resources to pay for their medications and co pays
The pay range for this position at commencement of employment is expected to be $130,000; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.
The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.
