Required Qualifications Active, full and unrestricted medical license in any U.S. state (North Dakota preferred) Board Certified / Board Eligible by the American Board of Urology Completion of an accredited Urology residency program (minimum 3 years surgical training) Current BLS and ACLS certifications Valid DEA license and NPI number Proof of malpractice insurance Clean licensing history (no suspensions, revocations, or restrictions) Documentation Requirements Candidates must provide: Copies of medical license(s) Board certification/eligibility documentation Degree(s) and accreditation(s) DEA certificate and NPI Malpractice insurance certificate Residency completion documentation BLS/ACLS certifications Annual OIG statement Note: Incomplete submissions will be considered technically unacceptable.
