• Legacy Behavioral Health Services is seeking a case manager to join our team in Lowndes County.
• Under general supervision performs a wide range of tasks in the community to assist clients in regaining control over their own lives while promoting the recovery process.
• Utilizing psychiatric rehabilitation interventions provides skills training, education and case management to clients and their families.
• Monitor client progress towards treatment goals.
• Extensive documentation required; accurately charts services and client progress.
• Must be able to work well with community agencies and function as part of a team..

The Case Manager will be supervised by the Clinical Team Leader.

Requirements

Completion of a bachelor’s degree in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling or an equivalent field AND one year of full-time experience in social services delivery. Knowledge of mental illness Valid GA driver’s license with less than 5 points
