The case manager performs community-based tasks to assist clients in their recovery process by providing skills training, education, and case management. They are also responsible for monitoring client progress toward treatment goals and maintaining extensive service documentation.
Candidates must hold a bachelor's degree in a relevant field such as social work, psychology, or nursing, along with one year of full-time experience in social services delivery. A valid Georgia driver's license with fewer than 5 points is also required.
The Case Manager will be supervised by the Clinical Team Leader.
Completion of a bachelor’s degree in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling or an equivalent field AND one year of full-time experience in social services delivery. Knowledge of mental illness Valid GA driver’s license with less than 5 points

While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, stooping and crouching as well as lifting and supporting the individuals served.
