The provider will deliver professional medical services, manage patient panels, and coordinate care within a patient-centered medical home model. They are also responsible for supervising support personnel and participating in quality assurance and performance improvement programs.
Candidates must hold a degree from an accredited Nurse Practitioner program and possess a current Arizona Nurse Practitioner license. A minimum of two years of experience in an ambulatory care setting is preferred, along with current DEA, CPR, and fingerprint clearance certifications.
