The Registered Nurse provides integrated physical and behavioral health care management for assigned consumers, including direct nursing care and treatment planning. They are responsible for maintaining medical records, coordinating care with professional networks, and responding to psychiatric or physical emergencies.
Candidates must hold an active Registered Nurse license in the District of Columbia and possess at least an Associate degree. A CPR/BLS certification is required, along with the ability to perform clinical assessments and maintain patient confidentiality.
$80,000 - $120,000 commensurate with experience
The Registered Nurse (RN) supports integration of Physical and Behavioral Health at Community Connections. The RN is responsible for Care Management for assigned consumers including: Teaming with internal and external providers, aggregating health data and ensuring medical record integrity, and providing direct nursing care that is emergent, urgent, and routine. The RN achieves key performance indicators: including service volumes, quality and timely documentation, beneficial outcomes as measured by health data and statistics, and teamwork.
