San Jose: North San Jose Center (In-Person Position)
FT, Monday-Friday, AM shift (9 AM-6: 30 PM)

Education/Experience: Applicants must meet any one of the following education and experience
Valid California Driver’s License and current automobile insurance and must maintain a driving record that meets the company driver's eligibility policy.
This position requires driving from home to home, working with clients in the community, and transporting clients in personal vehicles.
This position has primary responsibility for providing oversight and care management to the clients referred to Starlight programs. The Case Manager will provide services that may include case management/linkage, assessments, screenings, treatment planning/monitoring, and rehabilitation services to the clients and their families.
Focus on providing prevention and early intervention to individuals (10 to 25) who are at risk for experiencing early psychosis.
Working as a team to provide quality and comprehensive care to individuals and their families.
Sessions are in the community, clinic, home and some telehealth.
We provide services to individuals and families throughout Santa Clara County.
