The incumbent is responsible for delivering direct patient care in an ambulatory setting using the nursing process and independent clinical judgment. They also facilitate patient education, mentor staff, and participate in performance improvement activities to ensure quality outcomes.
Candidates must hold a current Registered Nurse license or temporary permit and obtain Basic Life Support certification within 30 days of hire. A Bachelor's degree in nursing is preferred, though an Associate's degree or diploma with enrollment in a BSN program is acceptable.
EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC). 2. Obtain certification in Basic Life Support within 30 days of hire date. EXPERIENCE: 1. Three (3) years of nursing experience. Assesses and facilitates coordination of multidisciplinary care across the care continuum. Provides communication, advocacy, and education in a culturally competent manner. 2.Clinically triages new patient referrals efficiently, to ensure timely access to care. 3. Collaborates with multidisciplinary care team and expedite sequence of patient's workup, active treatment, survivorship, and end of life care. 4. Provides communication and reinforcement of clinical information and serve as a conduit between patient and providers/clinicians to address needs of patients. 5. Performs targeted symptom assessment and management for identified patient population. 6. Assesses health literacy and educational needs of patient. Provides clinical education about diagnosis, treatment, side effects, and post- treatment care. 7. Assesses and documents patient's needs, goals, and preferences and work to ensure such are integrated into treatment and care delivery. 8. Bridges gaps in care and assesses/troubleshoots care transitions and barriers to care. 9. Documents all patient encounters in EPIC. 10. Assists with staff education, preceptorship, patient education, and participates in process improvement initiatives. 11. Participates in all service line team meetings.
