The RN Case Manager provides skilled nursing visits in patient homes, completing assessments and developing comprehensive care plans. They also collaborate with the interdisciplinary team, supervise support staff, and educate families to promote patient independence.
Candidates must hold an active, unrestricted Registered Nurse license, a valid driver's license, and CPR certification. At least one year of RN experience in home health or acute care is preferred.
