Bookmark Medical is seeking an experienced Primary Care Advanced Practitioner to join a well-respected and very busy community-based practice in Salem, New Hampshire.

  • Key Responsibilities
  • Provide comprehensive care to a diverse patient population, including children, adults, and geriatric patients
  • Conduct thorough physical exams, diagnose medical conditions, and develop personalized treatment plans
  • Promote health education and disease prevention
  • Order and interpret diagnostic tests and manage chronic conditions
  • Collaborate with specialists and other healthcare providers to ensure coordinated care
  • Maintain accurate up-to-date medical records and patient charts
  • Stay current with medical advancements and best practices in Primary Care

Minimum Qualifications

  • Graduate of an accredited school of nursing
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Current Nurse Practitioner or Physician Assistant certification
  • Comfortable using new technology and software
  • Minimum of one year of APP experience
  • Ability to work both independently and collaboratively within a team environment

Requirements
