Clinical pharmacists provide comprehensive pharmaceutical care by promoting safe, effective, and evidence-based medication use through therapy monitoring and collaboration with healthcare providers to optimize patient outcomes. Responsibilities include patient-specific education, documentation of interventions, adverse event reporting, and serving as an educational resource.
Candidates must possess a Doctor of Pharmacy (Pharm.D.) degree or a Bachelor's degree in Pharmacy with five years of relevant hospital experience, along with an active Illinois pharmacist license and Basic Life Support (BLS) certification. Board Certification in Pharmacotherapy (BCPS) is required, or must be obtained within 12 months of hire.
Education: Graduate of an ACPE-accredited School of Pharmacy required Doctor of Pharmacy degree (Pharm.D.) required Bachelor’s degree in Pharmacy with 5 years hospital based, patient care experience, considered in lieu of Pharm.D. Advanced Pharmacy Residency Training (PGY1/2) preferred Licensure/Certification/Registry: Licensed pharmacist in the State of Illinois required Board Certification in Pharmacotherapy (BCPS) required PGY-1 residency accepted in lieu of BCPS BCPS certification required within 12 months of hire Basic Life Support (BLS) required Experience: Minimum of three years relevant hospital based, patient care experience required Completion of a PGY-1 pharmacy residency program will satisfy this requirement
Work collaboratively with physicians, residents and other healthcare providers to promote and improve safe medication use Consult with physicians and other health care providers to develop and implement a medication plan that meets the overall goals of patient care established by the health care team Participates in interdisciplinary patient care rounds as appropriate and required In the absence of the lead clinical pharmacist, represents pharmacy at select interdepartmental meetings (i.e. Unit Base Council Meetings) as needed Promotes and demonstrates rational, cost effective drug therapy through effective, criteria based management of medications Performs the basic clinical and operational functions to care for patients on most medical units. Provide patient specific medication use teaching in high risk patients (i.e. Poly-pharmacy, pediatric patients, patients on high risk medications or who have been identified as high risk for readmission) Ensures appropriateness, effectiveness, and safety of each patient’s medication regimen in order to manage all aspects of medication use for patients on the units served Documents all recommendations and interventions in the electronic medical chart Prevent, detect, monitor, and report adverse events (medication errors and adverse reactions). Promotes evidence based, medication related care especially in the areas of Heart Failure, Stroke, AMI, Pneumonia, Sepsis and with use of all antimicrobial agents Provides patient education as needed and serves as a resource for accurate and timely Drug Information for all healthcare providers Serves as an educational resource for pharmacy colleagues, students, residents, physicians, nurses and other health care professionals. Performs order entry, order verification or clinical order review Actively participates in the professional development of pharmacy students and pharmacy residents as required Ensures compliance with all local, state and regulatory requirements and practices within Springfield Memorial Hospital’s policies and procedures. Assists lead clinical pharmacist or participates in at least 1 quality improvement initiative on an annual basis Maintains professional growth and development through continuing education, seminars, workshops and professional affiliations. Provide formal medication related consultations (i.e. Pharmacokinetic consultations) as required Performs other related work as required or requested.
