The Transitional Support Specialist provides personal care and support to clients in the TIP program, including supervising medication administration and daily living activities. They also facilitate skill development groups, maintain clinical records, and ensure the facility is clean and safe.
Candidates must have a High School Diploma or GED, with a Bachelor's degree in a human service field preferred. Experience working with severely mentally disabled populations and relevant certifications like CPR/First Aid are highly desired.
Harbor is seeking a Transitional Support Specialist to provide personal care services or accommodations to clients enrolled in the Transition to Independence Process (TIP) program. This can include planning and leading skill development groups, assisting with meal preparation, supervising the self-administration of medication and assisting with activities of daily living.
Position is full-time, 40 hours per week on 1st and 2nd shift. Schedule is Sunday, 1st shift from 6:45a - 3:00p, Monday - Wednesday 2nd shift from 2:45p- 11:00p, and Saturday 1st shift from 6:45a - 3:00p. Participation in the Thursday weekly staff meeting from 3p - 4:30p is mandatory.
