Administer nursing care to patients in their homes and coordinate care with the interdisciplinary team. Conduct comprehensive patient assessments and develop individualized plans of care to ensure quality end-of-life support.
Requires a current Registered Nurse license and at least two years of clinical nursing experience. Must possess reliable transportation and the ability to perform on-call duties.
Current state license as a Registered Nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license.
Two (2) years experience as a registered nurse in a clinical care setting. Hospice experience preferred.
Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Must read, write and comprehend English.
Reliable transportation and valid auto liability insurance.
