The Physical Therapist will perform functional assessments and design evidence-based treatment plans to help patients regain independence and mobility in their homes. They will also collaborate with an interdisciplinary team and supervise PTAs and Home Health Aides to ensure high-quality care.

$96,400.00 - $132,550.00/Annual Actual pay is determined based on job-related factors such as relevant experience, education, credentials, skills, internal equity, and business needs.

  • You’ll serve patients throughout the Warsaw and Lancaster County area by living out our mission — caring for others as we care for those we love — through:

  • Designing and carrying out evidence-based treatment plans that help patients regain independence and mobility
  • Collaborating with an interdisciplinary team including Nursing, Speech, Occupational, and Physical Therapists
  • Supervising PTAs and Home Health Aides to ensure consistency and quality of care
  • Building relationships that foster trust, confidence, and healing in every home

Qualifications

Education

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy (required)

Experience

• 1 year of Physical Therapy experience (preferred)

Licenses & Certifications

  • Active Physical Therapy License (PT) – Virginia Department of Health Professions (required upon hire)
  • CPR/BLS Certification – AHA, ARC, or ASHI (required within 30 days)
  • Valid Driver’s License (required; frequent travel using personal vehicle)

Requirements

Candidates must hold a Bachelor's, Master's, or Doctoral degree in Physical Therapy and possess an active Virginia Physical Therapy license. A valid driver's license is required for frequent travel to patient homes.
