The LPN will support clinical operations by performing patient intake, assessing symptoms, and coordinating care within a cardiology practice. They are responsible for assisting providers with medication administration, patient education, and maintaining accurate documentation in the EHR.
Candidates must hold a current Licensed Practical Nurse (LPN) license in Arizona or a compact state and possess at least one year of clinical experience. Proficiency in EHR systems, strong clinical assessment skills, and current CPR/BLS certification are required.
Phoenix Heart is seeking a Licensed Practical Nurse (LPN) to support patient care within our cardiology practice. This role is responsible for assisting with clinical operations, patient assessment, care coordination, and supporting providers in the management of patients with cardiovascular conditions. The LPN will play a key role in patient intake, symptom assessment, coordination of care, and ensuring timely communication between patients and providers to support quality outcomes and continuity of care.
Provide direct patient care under the supervision of physicians and advanced practice providers Perform patient intake including vital signs, medication reconciliation, and medical history updates Assess patient symptoms via phone and in-office and communicate findings to providers Prioritize patient needs and assist with scheduling urgent or same-day appointments as appropriate Manage inbound and outbound patient calls related to symptoms, medications, and follow-up care Support care coordination including follow-ups, referrals, and patient communication Assist with medication administration and treatments within scope of practice Monitor patient conditions and report changes or concerns to providers promptly Provide patient education on medications, treatment plans, and when to seek additional care Maintain accurate and timely documentation in the electronic health record (EHR) Ensure compliance with clinic protocols, evidence-based practices, and regulatory requirements Collaborate with providers, medical assistants, and administrative staff to ensure efficient workflow and patient care
