Licensed Practical Nurse (LPN) – Day Kimball Medical Group Family Medicine Full Time Day Shift (40 Hours) – Putnam, CT Day Kimball Health is seeking a Licensed Practical Nurse (LPN) to join the Day Kimball Medical Group Family Medicine team in Putnam, CT. ________________________________________ Overview The Licensed Practical Nurse (LPN) functions as an office nurse supporting daily clinic operations, patient care, provider workflow, patient communications, and clinical documentation. ________________________________________ Key Responsibilities Patient Care & Clinical Support · Document chief complaint and essential patient data, including height, weight, blood pressure, and head circumference · Assist providers during patient visits, procedures, and emergency situations · Assist patients to X-ray, Lab, or Emergency Department when needed · Set up nebulizer treatments and monitor patients · Assist with minor procedures, including PKU, sutures, and catheterization as needed Patient Communication & Documentation · Manage patient callbacks, test results, messages, and follow-up communications under provider direction · Complete medical record documentation and school or camp forms · Answer medical questions or refer patients to the physician or nurse practitioner when necessary · Reinforce education and health information with parents, guardians, and patients following provider consultation Medication, Immunization & Lab Support · Administer immunizations and allergy injections and document appropriately · Monitor medications, state vaccines, medical equipment, and supplies · Obtain strep cultures, document results, and ensure timely delivery to the laboratory · Retrieve special orders from Lab and X-ray as requested by the provider Office Operations · Maintain an even distribution of patients for assigned providers · Keep exam rooms stocked with medical and educational materials · Secure and restock exam rooms and laboratory rooms daily · Support daily clinic operations by maintaining supplies, equipment, laundry, and patient education materials ________________________________________
