Job Description: Summary
We are seeking a Physical Therapy Assistant (PTA) to join our Home Health Team in Lee County. In this role, there is a real impact in the comfort of patients’ homes. As part of our compassionate, high‑performing care team, you will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to improve mobility, strength, balance, and functional independence. You will collaborate with an interdisciplinary care team, educate patients and families on rehabilitation and injury prevention strategies, maintain accurate clinical documentation, and ensure compliance with all regulatory standards to deliver high-quality patient care and achieve optimal outcomes.

How will you make an impact & Requirements
Provide skilled therapy services under the supervision of a Physical Therapist.
Therapeutic
Interventions: Implement treatment plans, including therapeutic exercises, gait training, and assistive device training.
Patient
Education: Educate patients and families on techniques to improve functional skills and independence.
Collaboration: Work collaboratively with Physical Therapists, occupational therapists, and other healthcare professionals to ensure optimal patient outcomes.
Documentation: Maintain accurate and timely documentation of patient progress, treatment plans, and interventions.
Infection
Control: Adhere to strict infection control guidelines to protect both patients and healthcare workers.
Current and valid Physical Therapist Assistant (PTA) license in the state of Florida.
Experience: Minimum of 6 months in a hospital, skilled nursing facility, or rehab required.
Skills: Excellent communication, interpersonal, and critical thinking skills.
Professionalism: Strong work ethic, reliability, and a commitment to providing quality care.
Transportation: Reliable transportation to visit patients in their homes.
