In this position...
You will provide direct care, patient education, and the management of the applicable therapy (physical, occupational, or speech) called for in the patient’s treatment plan.
Consult with and educate other care team members concerning applicable patient assessment and treatment protocols.
Serve as a rehabilitation resource for the care team.
Evaluate the patient’s response to therapy services, assess service needs on an ongoing basis, arrange additional therapy services required, and obtain physician orders for such services.
Clinical record documentation demonstrates compliance with physician orders for care provided and the frequency of visits.
Ensure clinical documentation and Oasis time points are complete, accurate, and submitted with an error rate of less than 20%.
Serves as a preceptor to others when needed or requested.
Schedule: Full-time, Monday- Friday
Compensation: Pay Per Visit
