Team 5: Bohemia, Brentwood, Fire Island, Great River, Islandia, Islip , Islip Terrace, Lake Ronkonkoma, Oakdale, & West Sayville.
8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends Per Visits are required to work at least 1 shift per week Per visits are required to attend entire first week of orientation Monday – Friday (8:30AM -4:30PM.)
Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients. Measure vital signs, Administer IVs, change dressings, clean wounds as needed Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations. Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload. New York State RN license Minimum of one year of recent hands on experience as a Registered Nurse. CHHA experience strongly preferred Basic Life Support Certification Valid New York State Driver’s License and reliable transportation

Candidates must hold a valid New York State RN license and have a minimum of one year of recent hands-on experience. A valid New York State Driver’s License and Basic Life Support Certification are also required.
