The Patient Care Navigator oversees daily inbound communications and assists Care Managers in monitoring patient progress toward desired outcomes. They are responsible for resolving barriers to effective care, maintaining accurate documentation, and collaborating with the multidisciplinary healthcare team.
Candidates should have at least three years of experience as a Medical Assistant or Certified Nursing Assistant, with additional experience in a medical office or hospital setting preferred. Proficiency in Microsoft Office and knowledge of HIPAA regulations and orthopedic healthcare practices are required.
