The Medical Social Worker provides psychosocial assessments, counseling, and crisis intervention for patients and their families. They also coordinate discharge planning and collaborate with interdisciplinary teams to ensure continuity of care.
Candidates must hold a Bachelor’s degree in Social Work and be licensed or license-eligible. A Basic Life Support (BLS) certification and knowledge of community resources are also required.
Provides psychosocial assessment, counseling, advocacy, and coordination of community resources for patients and families. Supports continuity of care, discharge planning, and emotional well-being in the hospital setting.
• Hospital or healthcare social work experience preferred
