About the role

The Home Care CNA provides essential daily living assistance and restorative care to PACE participants in both home and day center settings. Responsibilities include monitoring health status, assisting with personal hygiene, meal preparation, and maintaining a safe environment for elderly participants.

The Home Care CNA is responsible for providing and assisting participants with activities of daily living and restorative/supportive care necessary to ensure the physical and health safety of the home environment. Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior Community Care of State (SCC). As assigned, works shifts in the SCC Day Center and in participant homes and provides care and services to participants and assists with the provision of a safe and effective day program.

    Essentials

    Consults each day with the Home Care Coordinator (or designee) to be updated on changes in participant status, changes in plan of care, and to confirm scheduled visits.

    Assists participants in the home with the following duties: 1. Personal hygiene such as skin, mouth, hair care and bathing. 2. Assists participants in and out of bed assisting with ambulation. 3. Assisting with prescribed exercise which participants and aide have been taught by appropriate personnel. 4. Prepares meals, including therapeutic diets, assists participants with meals. 5. Assist participants to the bathroom or in using commodes, bedpans, or urinals. 6. Assisting participants with medications which are ordinarily self-administered. 7. Other duties taught by a health profession for a specific participant such as: taking of vitals, changing colostomy bags, changing of non-sterile dressing and non-sterile bowel and bladder hygiene care.

    Performs directed household / homemaker services.

    Performs the following duties, as assigned or needed, in the

    Day

    Center: 1. Assists participants and or supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) in the Day Center as assigned or needed. 2. Monitors and assists participants with meals at feeding table. 3. Assists, throughout the day, in maintaining clean and orderly day rooms, kitchen and personal care areas. 4. Performs and or assists with laundry, including clothes for participants. 5. Assists the Drivers with the loading and unloading of participants from vehicles.

    Candidates must hold a current STNA license and CPR certification, along with at least one year of experience working with the elderly. A high school diploma, valid driver's license, and reliable transportation are also required for this role.
