The Hospice Nurse Practitioner performs face-to-face encounters to certify hospice eligibility and collaborates with interdisciplinary teams to ensure quality patient care. They are responsible for conducting physical assessments in various patient settings and maintaining accurate clinical documentation in compliance with regulatory standards.
Candidates must hold a Master’s degree or higher in Nursing and possess current state licensure as an Advanced Practice Nurse. A valid driver's license, national NP certification, and a strong understanding of hospice care principles and regulatory requirements are required.
