The Outreach Case Manager provides community outreach to identify, engage, and support homeless individuals or those at risk of homelessness. Responsibilities include linking clients to mainstream resources, housing, and mental health services while advocating for their needs within the community.
Candidates must have an MHRT/C certification or two years of social services experience if no degree is held. A bachelor's or master's degree in a related field is preferred, along with experience working with oppressed populations and a valid driver's license.
