The primary duties involve providing intermittent nursing services to home-bound patients, including comprehensive physical and psychosocial assessments, developing care plans, and coordinating all necessary services. Job Description

· Perform comprehensive assessments to establish an individualized care plan. · Facilitate communication between the patient and the interdisciplinary team. · Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence. · Utilize state-of-the-art technology to document skilled services provided.

Registered Nurse with Valid CT RN license One year of RN experience; Homecare experience preferred.

Valid driver’s license and reliable transportation required.

Computer proficiency required.

Requirements

Candidates must be a graduate of an NLN-approved school of nursing with a current Connecticut Registered Nurse license and at least one year of recent medical/surgical nursing experience, with community health experience preferred. Essential requirements include possessing adequate transportation with required insurance, the ability to perform physical demands like lifting and ascending stairs, and knowledge of professional nursing theory and agency procedures.
