The Behavioral Health Care Manager coordinates and tracks behavioral health services across primary care and community partners. They provide same-day consults, conduct screenings, and deliver evidence-based interventions to improve patient health outcomes.
Candidates must hold a bachelor's degree in a human services-related field or possess equivalent experience. Strong interpersonal skills, the ability to work in a multidisciplinary team, and familiarity with behavioral health conditions are required.
Bachelor’s degree in a human services-related field or equivalent experience Strong interpersonal skills with the ability to motivate and engage patients Ability to collaborate effectively within a multidisciplinary care team Comfort with data entry, documentation, and basic data tracking/analysis Working knowledge of behavioral health conditions and treatment approaches Familiarity with community resources and referral processes Preferred Competencies: Experience with brief behavioral health interventions (e.g., motivational interviewing) Understanding of integrated care or Federally QHC environments Ability to communicate effectively with patients, families, and clinical teams Strong judgment and ability to manage sensitive, confidential information Why SVCHS?
