The Primary Care Nurse Practitioner/Physician’s Assistant will provide primary care services to residents in skilled nursing and long-term care facilities. Responsibilities include performing physical exams, diagnosing and treating health conditions, and collaborating with facility staff.
Candidates must have an active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license, along with a state-specific license in good standing in New York. A minimum of 6 months of experience in long-term care or a skilled nursing facility is required.
