• Model, you will: Perform in-home assessments and urgent care interventions Work in partnership with physicians, nurses, and social workers Respond to patients flagged through remote monitoring and clinical team referrals Participate in patient discharge planning and community follow-ups Operate a fully equipped community vehicle (no ambulance) Deliver high-quality care under progressive protocols tailored to senior patients Conduct home safety checks, patient education, and wellness follow-ups The above are intended to describe the general nature and level of the work being performed by the person/people in this position. Make a Lasting Impact – See the same patients, build relationships, and make a real difference in their lives Qualifications Ohio Certified Paramedic (or eligible) Minimum 3 years of field experience (911 preferred) Excellent communication and independent decision-making skills BLS & ACLS (current) — AMLS, CT, CCP preferred Must pass clinical scenarios and PT testing Valid driver’s license with clean record
