The Hospice Registered Nurse Case Manager will build trusting relationships with patients and families to provide compassionate, high-quality end-of-life care. They will also collaborate with the care team and educate caregivers to ensure patient comfort, dignity, and quality of life.

Candidates must hold a valid RN license in the state of practice and possess at least two years of registered nursing experience, with hospice experience preferred. A current driver's license, CPR, and BCLS certification are also required.
