The Registered Nurse will provide professional nursing care to patients in their homes, managing interdisciplinary teams and developing individualized care plans. Responsibilities include performing comprehensive assessments, medication reconciliation, and coordinating with physicians to prevent hospital readmissions.
Candidates must hold a current North Carolina or compact RN license and a valid driver's license with insurance. An Associate Degree or Diploma in Nursing is required, along with 1-2 years of experience in med-surgical, critical care, or home health settings.
