The SUD Counselor conducts comprehensive assessments and develops individualized treatment plans to support patients in recovery. They facilitate counseling sessions and collaborate with interdisciplinary teams to ensure effective care coordination and service navigation.
Candidates must hold an active Substance Use Disorder Counselor certification (CADC or CATC) and possess at least one year of relevant experience. A valid driver's license and the ability to travel between clinical sites are required.

The Substance Use Disorder (SUD) Counselor reports to the Program Manager and provides counseling and care coordination services to individuals with substance use disorders. This role is responsible for conducting assessments, developing and implementing individualized treatment plans, facilitating individual and group counseling sessions, and supporting patients through recovery. The SUD Counselor collaborates with interdisciplinary teams and community partners to ensure comprehensive, patient-centered care.
