This is a hybrid role that requires fieldwork to visit patients’ homes in Kansas City, MO, and surrounding areas, with the possibility of occasional commuting to our Kansas City, MO office.
Personal Care Services is a caregiving arrangement in which a patient has a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in the patient’s home that will provide them with a range of care options as their needs evolve. The service is designed to provide options for alternative long-term care to persons who meet nursing facility-level care needs while preserving the dignity, self-respect, and privacy of the patient.
In this position, as a licensed practical nurse, you will perform clinical evaluations in the home of the Personal Care Services member to determine the level of service to qualify for the program. You will also conduct monthly visits with members who are On Care with FreedomCare to ensure they are receiving quality service and address any changes in their condition.
Provides support to the caregiver and member, and reviews services according to scope of the Personal Care Services program.
Respects client and employee rights and privacy, ensure the security of protected information, practices in an ethical manner and is compliant to the agency’s compliance program and privacy policy.
Follows the FreedomCare’s clinical policy and procedures.
Assesses and regularly reassess the client’s needs and their PCP summary form, treatment and education and communicates any changes to the member’s PCP.
Develops implements, evaluates, revises, and provides input to the client’s plan of care to include the physician’s plan of treatment, and problem list. Provides instruction of care to Caregiver according to the plan of care.
Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner.
Actively coordinates the care of the client through communication and written documentation to the physician, RN and Program Director, other disciplines involved in the client’s care and community resources.
Interprets the philosophy and objectives of the agency to clients, families, and others.
Performs other job-related duties as may be required or directed by the supervisor.

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $80,000 and $90,000 per year at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
Compensation Range $40—$45 USD
Requires a valid Registered Nurse license from the Missouri Board of Nursing and at least 2 years of clinical experience. Candidates must have a valid driver's license, an insured vehicle, and experience working with elders or adults with disabilities.
