The LPN will act as an active member of the interdisciplinary team to conduct clinical assessments, screenings, and treatment planning. They are also responsible for case management, discharge planning, and providing direct nursing care under the supervision of the nursing leadership.
Candidates must hold a current and active LPN/LVN license in Louisiana or a compact state and have completed an accredited LPN program. Additionally, current CPR and CPI certifications are required for the role.
Under the supervision of the Behavioral Health Director, House Supervisor, and/or Nurse Manager, the LPN shall act as an active member of the interdisciplinary team conducting at least, but not necessarily limited to; assessment and screening, development of nursing impressions and recommendations, treatment planning and treatment plan reviews, participation in team conferences, case management, discharge planning and placement, intake and referral and other clinical duties.
